Candidates will be referred in the following order: 1) Current, permanent employees of the Long Beach VA Medical Center, 2) Other current, permanent employees of the Department of Veterans Affairs, 3) Current, permanent employees of other federal agencies, 4) Preference Eligible Veterans, 5) All other qualified U.S. citizens.
Senior Social Worker performed assigned functions independently on station and in community, to include ability to drive a government vehicle to complete field visits, to assess all levels of potential safety risks, including environmental and patient population working with, which can be unpredictable and have ability to quickly modify interventions within their control, perform functions in a fast-paced, often non-traditional office setting, attain basic life safety certification annually and maintain their clinical licensure.
Senior Social Worker functions include: brief psychoeducational counseling or support, assist with inpatient and outpatient discharge planning needs, independently screen Veterans assigned for psychosocial needs via face-to-face or psychosocial/psychoeducational tele-health. He/she monitors the team assigned progress working to meet program performance measures. Senior Social Worker reviews cases the team presents and encourages the CM to consult for themselves and or bring to supervisor or homeless coordinator as needed. The Senior Social Worker carries their own case load. Other duties include but not limited to the following:
- Provides overall oversight for Veterans referred to HUD VASH are screened for eligibility. If meet requirements, conduct initial assessment, provide orientation, education on the documents needed to present to the PHA and educate the important for Veteran to attain required documentation within the designated time-frame of the performance measure, they assure Veterans enrolled are entered in HOMES and monitor and track Veterans during this time for progress, they provide or offer the Veteran support and resources to attain documentation. They assure orientation is documented, they notify the homeless medical clinic, the SUD social worker and Community Employment Coordinator of new Veterans enrolled; they educate Veterans on what to expect once voucher is attained and help the Veterans understand the team based CM.
- Assist individual Veteran throughout the application of a voucher/attaining voucher; conduct quality review of voucher applications before forwarding to the appropriate Housing Authority.
- Monitors the CM Acuity Level and assists the team in assuring the documentation supports the acuity level for Veterans assigned to the team and is reviewed weekly and recommends transfer upon need.
- Participates in a collaborative treatment planning with the Veteran, family and the multi/interdisciplinary treatment team.
- Assist in providing program specific orientation to staff newly assigned to the team; understand and demonstrate service recovery to resolve issues at the lowest level.
- Participates in PHA community meetings, as appropriate or when requested.
Work Schedule: Full Time, Monday - Friday, 0800 am - 4:30 pm
