The incumbent is accountable to the Deputy Chief, Quality, Safety and Value (QSV) Service and is responsible for working closely with the Services and Healthcare Groups Leadership to provide internal and external accreditation and site visit support (Office of Inspector General (OIG), The Joint Commission (TJC), Long-Term Care Institute (LTCI), Commission on Accreditation of Acute Rehabilitation Facilities (CARF). Is responsible for the following: monitoring, trending and analyzing data, interpreting TJC standards, interpreting VHA Handbooks and Directives regarding QSV/Performance Improvement/Patient Safety issues; collaborating with the facility Patient Safety Manager and Risk Manager; and monitoring progress of reviews and follow-up action items to ensure completion in a timely manner. The incumbent is responsible for utilizing a comprehensive, organized, continuous readiness program to review and identify opportunities for improvement in order to achieve excellence in the quality and safety of the care, treatment, and services provided to patients. The incumbent reviews all performance improvement data for assigned areas and assists responsible staff in initiating/facilitating appropriate action based on VHA guidance documents. The incumbent conducts various patient care studies based on areas assigned, including review of patient incident reports, medical record reviews for evidence of required documentation as well as reviews for the quality and safety of care, treatment, and services provided. The incumbent provides clinical expertise and consultation to assist in data driven decision making and performance improvement matters. The incumbent conducts tracers, operational assessments and participates in the design and analysis of special studies to define the cause and scope of issues or trends and assists in the identification of appropriate criteria for problem resolution. Analysis includes monitoring, tracking, and trending along with other statistically valid techniques to assist in focusing program efforts. The incumbent participates in the development of innovative approaches to the establishment, enhancement, modification, and/or improvements in a variety of facility programs, committees, and councils related to performance improvement/quality management.
