The Registered Nurse in the (MHICM) Mental Health Intensive Case Management Program has a case load of patients that are seen at least weekly primarily at home or in the community. Services provided include: transportation, shopping (food or clothes), delivery of medisets, filling of automated medisets, practicing social skills/leisure skills training, transporting/accompanying Veterans to medical or psychiatric appointments, teaching symptom management techniques, assisting with errands, arranging home maintenance as needed, treatment planning individually and as a team member, writing progress notes, administering and documenting injections, communicating with Primary Care provider and MHICM team for health care needs, and providing initial medical screening for Veterans admitted to the program as well as addressing acute and chronic medical concerns. At least 2 years of experience working with mental health patients is required.
The Registered Nurse is accountable for adhering to established policies, standards of care and standards of practice and for demonstrating leadership and improving care for a caseload of patients who have a diagnosis of severe mental illness with emphasis on coordination of care, and maintenance and promotion of optimal wellness across the continuum of care. He/She is responsible for conducting standardized assessments of patient's clinical status and providing these assessments to other treating providers, utilizing the nursing process in evaluation, problem identification, crisis resolution, treatment planning and evaluation of outcomes. The Registered Nurse in this role works in collaboration with other members of the multidisciplinary treatment team, under the direction of the MHICM Program Coordinator.
The Registered Nurse in MHICM will provide services to improve the quality of care delivered to veterans with a diagnosis of severe mental illness ranging in ages from 18 through the geriatric years taking into consideration their specific diagnosis, psychological, physical, spiritual and cultural needs. This care may be done in various settings including the patient's home, assisted living, group home, on the VA campus or other community settings as deemed appropriate. The RN will do the required VA driver's competency training to be able to transport patients. The Registered Nurse in MHICM will also provide coverage for a range of nursing responsibilities within Behavioral health.
The Registered Nurse in MHICM will be compliant with established MHICM performance indicators. The Registered Nurse in MHICM will be compliant with established policies and procedures that meet accreditation standards for both The Joint Commission and the Commission on Accreditation of Rehabilitation Facilities. The Registered Nurse uses professional standards of care and practice to evaluate self and others using professional standards, relevant statutes and regulations and takes action to improve performance.
