Job Description
Forms Coordinator/Management Analyst 2 - 26956
The Forms Coordinator coordinates and monitors form development, maintenance, and implementation to ensure compliance with polices, licensing and regulatory requirements. As a member of the Forms Committee, the Forms Coordinator is the subject matter expert on form development, identifying duplicate forms, retention and creating a communication plan to communicate updates and changes to form users. The Forms Coordinator collaborates with others to meet organization needs as related to forms management including the standardization of forms and their accessibility to staff.
This job will be located in Anoka, Moose Lake or St. Peter. Work location is negotiable based on the applicant selected.

Primary Responsibilities Include:
• Implementing and overseeing a standard process for form development
• Chairing Form Committee meetings including creating an agenda, ensuring meeting minutes are available and implementing decisions based on Forms Committee guidance
• Completing audits and reviews of forms to ensure the forms comply with policies, licensing, retention and regulatory requirements
Qualifications & Requirements
Must meet one of the following qualifications listed below:

• 1 year professional experience in record management or developing and managing compliant forms, OR

• Three years advanced technical or clerical paraprofessional experience in office administration that includes record management or developing and managing compliant forms OR

• A Bachelor’s degree in Public Administration, Project Management, Business Administration or related field and two years advanced technical or clerical paraprofessional experience in office administration that includes record management or developing and managing compliant forms
