Case Manager/Care Coordination
Responsibilities:
▪️ Provide intensive case management services to youth using a strength-based and comprehensive model.
▪️ Link children and families to resources and supports.
▪️ Monitor services, advocate for children and families, and assist with system navigation.
▪️ Develop and maintain relationships with families and community resources.
▪️ Assess program eligibility, identify needs, and link youth to services.
▪️ Monitor service quality and advocate for youth and families.
▪️ Assist with system navigation, permanency planning, and skill development.
▪️ Develop, monitor, and review service plans.
▪️ Provide 24/7 crisis intervention.
▪️ Create and manage discharge planning.
▪️ Support youth and families in spiritual and religious beliefs.
▪️ Promote professionalism through role modeling and respect.
▪️ Complete administrative tasks and reports.
▪️ Ensure compliance with regulations and standards.
Qualifications & Requirements
Qualifications:
▪️ Bachelor's degree in Human Services or related field.
▪️ Minimum 1 year of experience working with children and families.
▪️ Valid driver's license with a good driving record.
▪️ Ability to provide own transportation.
▪️ Meet auto insurance requirements.
▪️ Available for crisis response and non-traditional hours.
▪️ Certified Behavioral Health Case Manager (CBHCM) or ability to obtain.
Preferred Qualifications:
▪️ Experience working with children and families in community-based programs or Boys Town programs.
▪️ Bilingual skills.
