Specialist Practice Transformation Specialist
Job Requirements

I.General Summary


The Practice Transformation Specialist is a critical part of the team working with participating physician practices that are part of the University of Maryland Quality Care Network.The Practice Transformation Specialist serves as the key primary contact for assigned physician groups.In this role, the Practice Transformation Specialist will coordinate directly with the providers and office staff to identify opportunities for workflow improvement, integration of clinical and information technology resources, and facilitate training and educational sessions.

II.Principal Responsibilities and Tasks


The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification.They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.

  1. Manage practice transformation process for assigned practices to include but not limited to; practice observations of current state, review of findings, consultation on best practice recommendations and opportunities for implementation related to a practices readiness for value-based care.
  2. Responsible for specific areas of small to medium sized projects/initiatives within PHSO, may take on lead responsibility for smaller projects. Assists in the creation and planning of activities. Assists in the deployment and organization of resources for medium to large projects.
  3. Develop and deliver programs, training, and adoption to physician practices
  4. Comprehensive and accurate documentation and coding of risk-adjusted patient populations to ensure appropriate risk score and revenue capture
  5. Development and implementation of initiatives to improve performance against value-based contract metrics
  6. Coaching on pre-visit planning and closure of gaps in care
  7. Establishing strong personal relationships with Accountable Care Teams: physicians, nurses, administrative staff and management
  8. Provides assistance with quality assurance tools and processes.Seek an understanding of project/program interdependencies in order to effectively work with others on potential impacts.
  9. Keeping abreast of current industry, market, and policy landscapeUnderstands and articulatesthe programs business value and associated metrics.


Work Experience

Education and Experience

  1. Bachelors degree in Nursing, Business, IT, Health Care Administration or a related discipline; or equivalent combination of education certifications and directly related work experience is required.
  2. 3-5 years of client facing experiencein clinical integration or transformation activities, EMR and workflow development, assisting providers with value based payment transformation, or equivalent required.
  3. 3-5 years of experience working withphysician practices including; provider relations, provider education, practice transformation and/or practice management

IV.Knowledge, Skills and Abilities

  1. Demonstrate ability to think strategically. Demonstrate sufficient understanding of some functions to structure and manage project work.Generate innovative and practical solutions to complex or unusual problems.
  2. Highly effective verbal and written communication skills are necessary in order to work with all levels within the organization, and produce clear and concise reports of relatively complicated issues.
  3. Demonstrated change management and systems redesign expertise
  4. Demonstrated subject matter expertise in any of the following areas: Medicare advantage, Medicare Shared Savings Program (MSSP),managed care regulations, quality reporting,care management, population management, ACO strategy implementation and operations, and supporting providers in risk based contracts.
  5. Ability to operate in a fast-paced and dynamic environment
  6. Demonstrated understanding of healthcare metrics and use of analytical skills
  7. Extensive knowledge of the U.S. healthcare system
  8. Exceptional client-facing skills
  9. Excellent presentation skills; ability to engage with an audience and cater education/training to meet the needs and defined goals.
  10. Weekend, shift work, holiday, on-call, and may be require work beyond normal tour of duty to complete projects, meet deadlines, or respond to emergencies.

V.Working Conditions

  1. Working hours may require early or late hours (outside of traditional business hours) to meet program goals and support clinical schedules.
  2. Work is performed in various environments within UMMS.These environments include, but are not limited to the following:a standard office environment or Healthcare environment.
  3. Work may include regular travel to and from the various affiliated institutions within the Medical System and Quality Care Network.
  4. Work may be required in a Healthcare facility which the observance of Universal Precautions is mandated.Universal Precautions involves the wearing of protective clothing/equipment and the observance of safe work practice.
  5. Work is high demand and fast paced.
  6. Talking and hearing necessary for conversations with callers, visitors, patients, families and staff members.
